This looks identical to the knobs on the several series II 330 GTs that I've seen (including mine).

A conical lock nut holds the knob at the angle desired by the driver so his/her fingers fit into the grooves properly.

Another choice if you can determine where the loops go at each end is to take the cable to a good marine hardware store. They can swage fittings and connectors on almost any cable. These are the same fittings that hold masts up, so the strain from a Ferrari window winder shouldn't be a problem.

For the plastic vapor barrier behind the door panel, buy some clear heavy duty plastic (10 mil or so). It should be available at a building supply store and use double stick tape (office supply place) to go around the whole outside. Just be careful and check that it won't show once the door panel is...
